BRAF mutations are most commonly found in melanomas, but can occur in other forms of cancer. Not all mutations in BRAF cause cancer. The BRAF gene provides instructions to a protein responsible for managing important cell functions related to growth.

BRAF is one of the most frequently mutated oncogenes recognised in melanoma. The most frequent oncogenic BRAF mutations consist of a single point mutation at codon 600 (mostly V600E) that leads to constitutive activation of the BRAF/MEK/ERK (MAPK) signalling pathway.

The V600E mutation occurs in between 70–90% of patients with BRAF-mutant melanoma. The V600K is the second most common (10–30%), occurring more frequently in older patients and those with chronic sun-damaged skin.

BRAF-V600 mutations occur in approximately 50% of patients with metastatic melanoma. Immune-checkpoint inhibitors and targeted therapies are both active as first-line treatments in these patients regardless of their mechanisms of action and toxicities. However, an upfront therapeutic strategy is still controversial.
